The importance of Tova leading Jeff toward the black horse is showing that she has already accepted the fact that Jeff is going to die. Since she already knows that he is going to die, she has no problem leading him to the black horse, which represents death. It is probably better that she has accepted his death because now she can just concentrate on letting him be happy for the remainder of his life.
