REPORT OF CULMINATING EXPERIENCE

Purpose of the Form:

  • This report certifies that a graduate student has successfully completed all components of the Culminating Experience for the designated degree as specified in the Catalog;
  • This form is to be filed by all graduate students who are applying to graduate and will be kept by the Evaluations office as part of the student’s permanent file.

GRADUATE ACADEMIC PETITION

Purpose of the Form:

  • Amendment to Master’s contract (e.g., change in unit requirement for a class, add/delete courses listed on the contract, transfer of units from another University if not already listed on a Master’s contract);
  • To substitute a course on a Master’s contract for another course applicable to the Master’s program;
  • To request a Waiver of Residency if not enrolled the quarter of graduation (Please note: the waiver of residency is not automatically approved. Student needs to show strong justification for not being enrolled the quarter of graduation. Student must be in active status to be considered for waiver.
  • Change of Status from conditional to unconditional standing or dismissal from degree program.
  • To request a recertification for courses more than 7 years old. Please click on Supplemental Information to review some helpful guidelines. Each student should check with their department and/or graduate coordinator to find out what the College policy is for certification of outdated coursework.

PETITION TO CHANGE/ADD GRADUATE DEGREE OBJECTIVE

Purpose of the Form:

Continuing graduate students must file this form when:

  • Changing from one major field to another for the Master’s degree;
  • Changing from a certificate/credential objective to a Master’s degree objective
  • Adding a Master’s degree to a credential program
  • Changing from no objective to a stated master's degree objective.

REQUEST BY AN UNDERGRADUTE FOR GRADUATE/UNDERGRADUATE CREDIT

Purpose of the Form:

Undergraduate Students file this petition to request to receive graduate credit in 300, 400, and 500-level courses or undergraduate credit in 500-level courses.

GRADUATE ASSISTANT/TEACHING ASSOCIATE (GA/TA) FEE WAIVER APPLICATION

Purpose of the Form:

Eligible graduate students working as a Graduates Assistant or a Teaching Associate must file this application to receive University state tuition reimbursement.

UNIVERSITY FORMS

LEAVE OF ABSENCE

Purpose of Form:
To be used to request an absence of more than two consecutive quarters, but no more than eight consecutive quarters

REQUEST FOR GRAD CHECK

Purpose of Form:

Graduate students are required to file for a graduation check the quarter prior to graduation.  The graduation check is designed to summarize the requirements remaining for the completion of the Master’s Degree. http://www.dsa.csupomona.edu/registrar/Forms.asp

APPLICATION TO GRADUATE

Purpose of Form:

A graduate student who expects to receive a degree at the end of any quarter must complete an application for graduation in the Registrar's office prior to the deadline listed in the academic calendar. The student must be enrolled in the University the quarter he/she graduates. Students seeking a master's degree will be held responsible for meeting requirements applicable to the program of their choice and for fulfilling general master's degree requirements.

IMMUNIZATION REQUIREMENT FORM

Purpose of Form:

The California State Legislature has mandated that, as a condition of enrollment at Cal Poly Pomona, all newly enrolled students must meet certain immunization requirements. Students born on or after January 1, 1957, and who did not graduate from a California high school on or after January 1, 2005, must provide proof of immunization from Measles and Rubella.

Proof of immunization(s) must reach Student Health Services, Bldg. 46 before second quarter registration of classes or a hold will be placed on the student’s record.
