Memorandum of Understanding

Permission to Have an Animal in a University Activity

California State Polytechnic University, Pomona (University) approves the request of ___________________________ (Individual) to have the following animal ______________________ be physically present in the following University building ________________ on _____________________. This approval is contingent upon the Individual agreeing to the following:
 
  1. Individual agrees to assume all risk of injury, of every kind and nature, which may result result from the participation of Animal in the Activity.

    2.  
  2. Individual agrees to indemnity and hold harmless and completely release the State of California, the Trustees of the California State University, California State Polytechnic University, Pomona and the officers, agents and employees of each from all claims or causes of action arising out of, or in any way connected with Animal's participation in the Activity.

  3. Individual understands that the University does not assume any liability or responsibility for the acts of Animal while participating in the Activity.

  4. University has provided Individual with a copy of University's Animal Policy. Individual agrees to abide by University's Animal Policy.

  5. Individual agrees that Animal shall at all times be under his/her control and when not under his/her control shall be confined to a kennel.

Individual understands that failure to comply with University's Animal Policy or failure to comply with the terms of this Memorandum of Understanding may result in Animal being excluded from participation in any future University activity or from being allowed to be present in any University building.

  7. Upon request by University, Individual agrees to immediately remove Animal from University premises if University deems the Animal to be disruptive to the orderly operations of University. Individual further agrees that if he/she fails to comply with such a request from University, University has the right to immediately remove Animal from University premises.
