It is important to update your personal information when changes occur. Should your name change, you will need to update your record with the University for employment and payroll purposes.
To file a name change, you will need to first update your record with the Social Security Administration and obtain a new Social Security Card that reflects your new name. Federal and State regulations, as well as CSU policy, require that we validate that each employee's name and social security number on record exactly matches the name and social security number that appears on the individual's Social Security Card to ensure proper tax reporting.
Once you have a Social Security Card which reflects your new name, you will need to complete the Employee Action Request (EAR) if you are faculty/staff, or the CSU Student Payroll Action Request ( SPAR) if you are a student assistant. A Customer Service Center Representative , available by telephone at extension 3733, can help you identify which form to use.
To file your name change, please take your completed form to the Customer Service Center, located on the ground floor of the C.L.A. Building 98, Room B1-20. You will need to bring your Bronco Card for identification and your new Social Security Card for verification purposes.
Important Notice: EAR and SPAR Forms cannot be filed through the mail, electronically, or by fax--you must submit the EAR or SPAR in person to the Customer Service Center and present your Bronco Card for identification purpose and your new Social Security Card so we may verify that it matches your name as indicated on your name change request.
