Lending Policy - I-Poly High School

    Parental authorization via the contract must first be on file at the I-Poly office prior to receiving Library privileges.

REGISTRATION

You MUST have your I-Poly/Library photo ID and register at the fourth floor Circulation Desk. Your Cal Poly barcode is for your personal use ONLY. When you accept your barcode, you assume the responsibility for all materials that are checked out. You must carry your I-Poly ID/Library card at all times while visiting the library.

CHECKOUT

The registered I-Poly photo ID card is required when checking out any material.

CONDITIONS

A limit of (5) five items for a maximum of (2) two weeks may be checked out. Books can be renewed twice in person with or without book(s) in hand, provided there are no holds on them. Reserve and Periodicals material, all service counters, CD-ROM reference database, and microform reader/printers are all available. NOTE: Cal Poly students will have priority at the Reference, Reserve, and Periodicals service counters, CD-ROM reference database, and microform reader/printers. NOTE: The I-Poly Photo ID card is not-transferable.

The following services are NOT available to I-Poly students: Mediated online searching, reference assistance by appointment, document delivery, or software.

LOST BOOK REFUNDS

The replacement fee is refundable, minus a $6.60 processing fee, if the book is returned in good condition and within a one-year of payment date.

OBLIGATIONS

The overdue fees are: for Circulation, $.15 cents per item per day. For Periodicals, $.50 cents per item per hour or $1.00 per item per day, and Reserve ranges from $.15 per item per day, $.50 cents per item per hour or $1.00 per item per day, depending on materials borrowed. Fines must be paid when the accumulated fine record reaches $5.00. The replacement fee, a processing fee and billing fees will be charged for unreturned or lost materials.

PENALTIES

Failure to return overdue materials and/or pay fees, will result in temporary loss of borrowing privileges. Repeated or serious infringements may result in permanent loss of borrowing privileges. The I-Poly High School will be notified and held responsible for its delinquent patrons.

ATTEMPTED THEFT

In an attempt to limit unauthorized use of library materials, library personnel will record the name and identification number of anyone attempting to leave the library without properly checking out materials. Students who are repeat offenders will have their history forwarded to the Director of Judicial Affairs, who will investigate each reported case for potential disciplinary action, up to and including suspension or expulsion from the California State University system.

EXPIRATION

June 30th of each year.
