Application Check List

All application materials listed under “A” below should be sent to the following address:

  1. Your package should be received no later than January 15th. Please refer to this website for more information on process and content:  http://www.csupomona.edu/~la/mla_apply.html.  Send the following materials to the Department at the address above.
  2. Your letter of intent
  3. Your written response to the questions about the greatest challenges facing society today
  4. Your portfolio (10 to 40 sheets, 9” x 12” maximum, creative works of any type)
  5. A copy of the application you submitted to the university or a department record of application
  6. Two letters of reference
  7. Copy of Transcripts (does not have to be official)
  8. Copy of your notice of GRE scores
  9. Copy of you notice of TOEFL scores (international students only)

B. Online at http://www.csumentor.edu/AdmissionApp/ please complete the official Cal Poly Pomona application.  Your undergraduate university should send one copy of an official transcript directly to: Office of Admissions and Outreach, Cal Poly Pomona, 3801 W. Temple Ave., Pomona, CA 91768.  The Cal Poly Pomona Federal School Code is 001144.
