How to Report Academic Dishonesty & Disruptive Behavior

How to Report Academic Dishonesty

Thank you for taking the time to report a violation of the Student Conduct Code.  Executive Order 1073 states that all academic dishonesty cases that occur in the classroom shall be handled by faculty members.  Faculty must then report the incident to the Office of Judicial Affairs.

To report an Academic Dishonesty Incident please send an email to judicialaffairs@csupomona.edu, including the following information:

1. Faculty / Staff Information

First Name

Last Name

Department

Extension

CPP Email Address

Class Subject & Catalog Number (In which incident occurred)

2. Student Information

Bronco ID

First Name

Last Name

3. Incident Information

Date of Incident

Description of Incident

What actions have you taken?

Do you recommend further action from the University?

If you are requesting further action from the University with regards to this academic dishonesty case, please submit copies of any relevant materials to the Judicial Affairs Office, Building 26-110 or email materials to judicialaffairs@csupomona.edu.

 

How to Report Disruptive Behavior

To report an a Disruptive Behavior Incident which occurred in a classroom please send an email to judicialaffairs@csupomona.edu, including the following information:
