Welcome to Cal Poly Pomona...
The New Employee Benefits Orientation is provided to help guide new and/or newly eligible employees through the process of choosing benefits through Cal Poly Pomona.
Orientations are conducted monthly to assist eligible employees in making an informed decision regarding their benefit choices. Following is the current orientation schedule. Employees eligible for benefits (see Benefits Eligibility for details) who have not been scheduled for an orientation, should call extension 3734 (or if off campus, [909] 869-3734) or E-mail nskitamura@csupomona.edu for a reservation. Attendance at a Benefits Orientation is not mandatory to enroll in the benefits program; however, it is highly recommended.
To enroll in benefits, newly eligible employees should complete a Benefits Enrollment Information Worksheet and submit it to the Benefits Section of Human Resource Services within 60 days of the effective date of their qualifying appointment. If an employee submits the completed request by the end of the pay period in which their qualifying appointment begins, medical coverage may begin the first of the following month; however, dental, vision and/or FlexCash will not begin until the first of the second month. The coverage effective date will be delayed if the Benefits Enrollment Information Worksheet, including all necessary enrollment information, is not submitted in a timely manner.
As a reminder, employees attending the New Employee Benefits Orientation should bring their new employee benefits folder to the orientation. Also, if an employee is including a spouse on the medical enrollment, the spouse's social security number and a photocopy of the marriage certificate is required and must be attached to the Benefits Enrollment Information Worksheet.
