Following is a list of voluntary insurance plans available to eligible employees who wish to purchase the insurance and have premiums set up as a monthly payroll deduction. Brochures for each plan are available from the Customer Service Center located in Human Resource Services, CLA Building 98, Room B1-20. Note: Employees have sixty days from the date of the appointment which qualified them for enrollment in benefits to be eligible to purchase the Standard life insurance without evidence of insurability.
