Application Information

APPLICATION PROCESS

The Cal Poly Pomona Application ( Word format) and ADA Reference Forms (PDF format) may be downloaded from this website. Please note that the application is slightly different than the ADA Application form. It is to your advantage to use the application form specific to Cal Poly Pomona, however the ADA Form will be accepted.

The Cal Poly Pomona Dietetic Internship participates in the matching process through D&D Digital. Please see directions below.

OTHER NEEDED FORMS

Since we have a large number of individuals who read your packet, it is necessary for us to require a signed release allowing the packet to be circulated. The Release Form is available from this website. Please include it in your package.

If you have completed your degree more than five years previously, include the Recency of Education Form, also available on this website.

 

APPLICATION PACKAGE

The application packet must be postmarked by February 15, 2008 and MUST be complete when received by the Program Director. Materials, including recommendations, sent under separate cover, and incomplete packages will NOT be accepted. Photocopies will not be accepted; all materials must be original.

Please use the checklist below for material submitted to Cal Poly Pomona Dietetic Internship. All application packets must include the following:

$50 processing fee, made out to HNFS Department, Cal Poly Pomona.
Application Form, typed and complete. Please include your email address.
Authorization for packet review.
Verification Statement OR Intent to Complete.
One set of SEALED, official transcripts from all Colleges or Universities attended.
SEALED and SIGNED recommendations (3 required), on Recommendation Form; at least one from a Registered Dietitian and one from a faculty member in the department where you have received your Verification Statement.
Letter of Application, 1 to 1 1/2 pages which introduces yourself, includes short and long-term goals, why you decided to apply to the program at Cal Poly Pomona, how this program meets your goals, and other information pertinent to selection.
Recency of Education Form, if applicable.

 It is suggested that you do not use UPS, since they do not place a postmark date on the envelop. Express Mail, Fed Ex or any method that guarantees arrival to us within three to four days past the postmark deadline is best.  Please check other mailing options so the postmark date is readily visible. Registered and regular mail may take up to 2 weeks to arrive. Select your mailing option carefully.

MATCHING INFORMATION

All applicants to Dietetic Internships (DI) must participate in computer matching. Applicants should request instructions and a mark/sense card to prioritize their DI. Applicants should request this material from any ADA-Approved Didactic Program in Dietetics or from D & D Digital Systems. This request should be made to allow turn around time for submitting by the February 15, 2008 postmark deadline. There is no charge for this material. However, there is a $50.00 charge for computer matching that is due with the applicant's prioritized ranking.
