Make up Test Information

Make up examinations and off campus examinations may be scheduled at the Test Center. Students of Cal Poly Pomona must be experiencing a hardship such as; having missed their test due to an emergency or a scheduling conflict with another test. All accommodations for disabilities must be processed through the Disability Resource Center (DRC). Students must obtain instructor approval and then request an appointment through the link listed below. Requests are approved or denied within one business day.

• Faculty please review the Proctor Policy (PDF) and submit a completed Test Proctoring Form from the Forms page along with all test materials to the Test Center.
• Students requiring make up examinations may use the link shown below to schedule a test time. You may check back under the same link in the morning of the next business day to see if your appointment has been approved. Hours of operation are on the Test Center Website Home Page.
  
  Appointments will be denied if the Test Center has not received the test and Test Proctoring Form from the course instructor.

• Students wishing to take off campus examinations must contact the Test Center via phone at (909) 869-3353. An appointment for an off campus examination will not be scheduled until the exam and all relevant materials are received.
