Purpose of Form:
To be used to request an absence of more than two consecutive quarters, but no more than eight consecutive quarters

Students in all graduate programs (except Masters of Arts in Education) do not have to complete a Request for Graduation Check form. In lieu of having a graduation check, students will be able to access their Degree Progress Report. Once the approved Program of Study "Contract" has been received in the Registrar's Office from the Graduate Studies Office, it will be reviewed by an evaluator and noted on the DPR as the graduation check.

A graduate student who expects to receive a degree at the end of any quarter must apply for graduation online through Bronco Direct prior to the deadline listed in the academic calendar. The student must be enrolled in the University the quarter he/she graduates. Students seeking a master's degree will be held responsible for meeting requirements applicable to the program of their choice and for fulfilling general master's degree requirements. The Research Certification Form is be filled out by student and his/her commitee chair before collecting any data for the student's thesis or project which may involve the use of humans and/or animals in some way.
The California State Legislature has mandated that, as a condition of enrollment at Cal Poly Pomona, all newly enrolled students must meet certain immunization requirements. Students born on or after January 1, 1957, and who did not graduate from a California high school on or after January 1, 2005, must provide proof of immunization from Measles and Rubella.
Proof of immunization(s) must reach Student Health Services, Bldg. 46 before second quarter registration of classes or a hold will be placed on the student’s record.
