The Appointment Authorization and Effort Certification form - Excel
This form is used for all university faculty and staff members who work 10 hours or more a month as cost sharing on the TQE/Teacher PREP Grant.
You can obtain the following information from your college’s Administrative Analyst Specialist:
The form must be signed by both the employee and immediate supervisor/dean. Forward to mail location: TQE/Teacher PREP (NASA).
In addition, please fill out the Contributed Services Description and Documentation - Doc
form to indicate the type of activities or services rendered.
Attach all back-up documentation for cost sharing (receipts, invoices, etc.).
In-Kind Time Sheet form - Excel
This form is used for all university faculty and staff members who work less than 10 hours a month as cost sharing on the TQE/Teacher PREP Grant.
This form is also used by all non-university personnel to document the number of hours and type of work that benefited the grant project.
The form must be signed by both the employee and immediate supervisor. Third-party partners sign the form and then forwarded to the TQE/Project Director -- mail location: TQE/Teacher PREP (NASA), Suite 2340A, 3801 Temple Avenue, Pomona, CA 91768.
Attach all back-up documentation for cost sharing (receipts, invoices, etc.).
