Math and Science Teacher Education Recruitment, Retention program at Cal Poly Pomona

Please complete all sections of the MaSTER² Teacher Participation Form.  For questions please contact Dr. Laurie Riggs in the Department of Mathematics and Statistics (lriggs@csupomona.edu) or the MaSTER² office at (909) 869-3480.  Send the Teacher Participation Form and Reimbursement Form (if necessary) to:

Note: All reimbursement requests are to be accompanied with the original receipts.  The following is a breakdown of some required receipts and/or information.  For each category,

• Travel - Conference fee, Registration information, Teacher's name, Address
• Substitutes - Teacher's name, Date of service, Reason for substitute
• Stipends - Teacher's name, Address, Social security number
• Instructional Supplies - Original receipts of purchased items
• Tuition - Teacher's name, Address, Social security number, Copy of transcript, Receipt of payment
• CSET Test -  Teacher's name, Address, Test date, Receipt of payment

If payment was made with a personal check or credit card, a copy of the cancelled check (front & back), or a credit card statement copy will be accepted.  Any personal information, i.e., bank account # or credit card # can be whited out.  For more information, please contact Ms. Cidni Griffith (office administrator for the MaSTER² program) at (909) 869-3480 or cgriffith@csupomona.edu.
